Insulin and Multi-day trips

So, I'm looking for thoughts and advice on how easy it is to manage on a longer trip with a medication (for example insulin) that should be kept refrigerated.

Is it possible? In summer? In winter? In the rockies? Tips? What do you do?

One of my ski friends has an insulin pump for diabetes. He seems to manage fine summer and winter, but, now that you mention it, I'm not sure what he does, apart from change the pump every three days and refill with insulin. So, possible, but, I'm not sure on the details. I could find out...

Cool! Good link. Sounds great for the mountains. The problem is part of this question relates to a 6 week kayaking trip in Thailand where I expect there will be no cold water.

Sandy, thanks for the offer but I was just using insulin as an example as it's probably the most common medication that needs to be refrigerated and taken on trips. It's not actually for diabetes so there isn't a pump option in this case.

I've never had to keep medication refrigerated but have successfully kept cheese cool enough to never sweat by packing right up against my water in my pack, then transferring to the bear bin once it's cool in the evening, and removing it again in the morning before the sun hits it.
but I'm regularly filtering icy water into my water bladder. i don't have a good suggestion for Thailand.
